You need to provide this information so we can contact you and book your appointment. If you choose not to provide these details, we may not be able to offer you our services.
If you are a health provider sharing patient information with Gippsland Wellness Centre to make an appointment, you must have received permission before providing their details. This ensures we respect the privacy and consent of all individuals whose information is collected.
